Process for Requesting Volunteer Clearance:
PLEASE PLAN AHEAD! This process could take 2-6 weeks to complete.
STEP 1: Complete Application, Code of Conduct, and Request for Live Scan Form.
STEP 2: Submit below items to volunteers@ouesd.k12.ca.us:
Copy of Driver's License or Identification Card
TB Clearance Documentation (Negative TB skin test, TB risk assessment certificate, or healthcare provider statement verifying no active TB.)
Required initially, and renewed every four years.
STEP 3: Schedule an appointment for Live Scan clearance:
Call 925-625-0700 x4051 to schedule a fingerprint appointment.
If there is no answer, please leave a message.
If you are unable to email the required items, please bring them to this appointment.
Request for Live Scan Service Form.
If you are not using the district Live Scan service, please take this completed form to a local Live Scan location. The district provides this service free of charge, local companies will charge a processing fee.
STEP 4: Complete Mandated Reporter Training (Required Annually):
Once your FBI/DOJ Live Scan clearance is received, we will email you the Volunteer Mandated Reporter Training link.
If already completed for your place of employment, please provide your certificate of completion.
Approved volunteers will be contacted to pick up their volunteer badge at their school site.
Please report lost or stolen badges immediately to your school front office.
Replacement badges may incur a processing fee.
Replacement badges may take up to 2 weeks to process.

STEP 5: VOLUNTEER DRIVERS ONLY (Required Annually):
To transport students (other than your own) for field trips, complete the Driver Transportation Form and submit all required documents to your site Principal for approval (at least 15 days before the trip).
To transport your own student, complete the Transportation Exemption Form and submit it to your site Principal for approval.
